Please arrive 10 minutes prior to your first appointment to complete the registration form.
For the first time registration, please bring
Valid Medicare Card and Photo ID. Please also bring your DVA Card, Health Care Card or Concession Card if you have one.

- Routine preventative health checkups (eg. 45-Years old Health Assessment, Heart Health Assessment)
- Medical reviews (eg. Diabetes review)
- Screening (eg. Cervical Cancer Screening, Mammogram)
- Immunisation (eg. Routine Childhood Immunisations)

Request for Scripts, Medical Certificates and Referrals
Please make an appointment for repeat scripts, medical certificates or referrals. Please be informed that medical certificates cannot be backdated.
Privacy
Nicholson Road Medical Group is committed to protecting our patients’ privacy according to the Privacy Act.
The practice uses a computerised, paperless system. Access to computer records containing your medical information is restricted to medical staff.
All hard copies of patient documents disposed of in a secure manner, after scanned and stored in the patient medical record database.
A hard copy of our Privacy Policy is available on request at the reception.
