Nicholson Road Medical Group warmly invites all patients dealing with Type 1 and Type 2 Diabetes to attend our specialised diabetes care. Our aim is to enhance your understanding of diabetes and empower you to better manage your condition.
Here's what you can expect:
Initial Consultation:
During your initial visit, we will complete your Diabetes Cycle of Care assessment.
Follow-up Appointment (3-6 months later):
Return for a comprehensive review and additional assessments, which may include GP and Practice nurse assessment, ECG, GPMP/TCA evaluations, and referrals to Allied Health professionals and Endocrinologist as needed. Our dedicated team will discuss the various options available to address your specific needs comprehensively.
DIABETES CYCLE OF CARE:
CLINICAL ASSESSMENT:
Weight, Height, and BMI Measurement: Every six months, twice during each cycle of care.
Blood Pressure Measurement: Every six months, twice during each cycle of care.
PATHOLOGY TESTS:
HbA1c Assessment: Once annually to evaluate diabetes control.
Total Cholesterol, Triglycerides, and HDL Cholesterol Check: Annually.
Microalbuminuria Test: Annually.
ALLIED HEALTH ASSESSMENT:
Comprehensive Eye Examination: Every two years by an Optometrist or Ophthalmologist.

Facts about diabetes
1) Diabetes is the fastest-growing chronic condition in Australia. Currently, almost 1.1 million Australians have been diagnosed with diabetes.
2) 280 Australians are diagnosed with diabetes daily. Over 100,000 Australians were diagnosed with diabetes in the past year.
3) Additionally, for each individual diagnosed with diabetes, there is often a family member or caregiver who plays a crucial support role. This means that approximately 2.2 million Australians are affected by diabetes daily.
4) The total number of Australians with diabetes and pre-diabetes is estimated to be 3.2 million.
5) Indigenous Australians are three times more likely to have Type 2 diabetes compared to non-Indigenous Australians.
6) At least 2 million Australians have pre-diabetes and are at high risk of developing Type 2 diabetes.
